		<title>TML IEBP to Launch Healthy Initiatives Program</title>

		<content:encoded><![CDATA[<P>TML IEBP is excited to launch a new suite of health improvement programs for all TML IEBP members. The Healthy Initiatives program will be available to all covered individuals beginning October 1, 2007. It contains a wide array of health improvement tools including:<BR>
<OL>
<LI><B>Community-Based Biometric Screenings</B> - TML IEBP covers many biometric screenings performed by Network providers at 100%. For more information about preventive benefits and biometric screenings, please consult your Schedule of Benefits. <BR><BR>To help you keep track of your biometric data, TML IEBP has created the <A href="http://tmliebp.org/pdfs/HI_EE_Wellness_Scorecard.pdf" target=_blank>Personal Health Record</A>. This form helps members keep track of recommended tests and screenings. The Personal Health Record is a key component of the Healthy Initatives Incentive plan and is also available as a handy <A href="http://tmliebp.org/pdfs/HI_EE_Wellness_Wallet.pdf" target=_blank>Wallet Card</A>.<BR><BR>In addition to the Personal Health Record, TML IEBP has developed the <A href="http://tmliebp.org/pdfs/HI_Provider_Wellness_Scorecard.pdf" target=_blank>Provider Billing Tips</A> sheet. Take this document with you on your next provider visit to help your doctor understand how to bill biometric screenings covered by the Healthy Initiatives program. <BR><BR>
<LI><B>Health Power Assessments (HPA)</B> - Health Power Assessments are an important tool for identifying health risk factors. TML IEBP members will have access to an online Health Power Assessment tool through the new TML IEBP Web Wellness Portal. In addition to the online version, members can complete the HPA at Health Fairs or over the phone with the Medical Management department. The Health Power Assessment is a key component to the Healthy Initiatives Incentive Plan. More information about the Web Wellness Portal and the Healthy Initiatives Incentive Plan is found below.<BR><BR>
<LI><B>Multi-Disciplinary Medical Management</B> - TML IEBP employs a Multidisciplinary Medical Management team specially trained to help members with healthcare access and health improvement. The Medical Management team uses advanced claims stratification techniques to determine which covered individuals are most at risk for chronic health problems. Medical Management combines this data with the Prochaska Model in order to identify individuals&#039; readiness for change. Personal Health Coaches from the Medical Management Team contact covered individuals several times per year in order to encourage healthy living. <LI><B>Web Wellness Portal</B> - The TML IEBP Web Wellness web portal is an online health improvement tool that will be available to all TML IEBP members beginning October 1, 2007. This portal contains an online Health Power Assessment, e-learning lessons, a goal tracker and a vast medical knowledgebase. The web wellness portal is available to all TML IEBP members with Medical benefits. <BR><BR>To access this portal, login to the myTML IEBP website and click the "Web Wellness Portal" link found in the "Healthy Initiatives" menu on the left navigation panel. The first time you login, you will be asked to secure your account by providing an email address and your mother&#039;s maiden name. Once logged in, you&#039;ll gain immediate access to all of the useful tools in the Web Wellness Portal.<BR><BR>
<LI><B>Healthy Initiatives Incentive Plan</B> - The Healthy Initiatives Incentive Plan is an opportunity for TML IEBP Members to receive a monetary reward for completing recommended screenings and preventive tests. Each covered individual over the age of 18 will be provided with a <A href="http://tmliebp.org/pdfs/HI_EE_Wellness_Scorecard.pdf" target=_blank>Personal Health Record</A> at the time of enrollment. Covered individuals can also access an interactive version of the Personal Health Record inside of the Web Wellness Portal. The Personal Health Record contains a list of preventive tests that are age and gender specific. Members that complete all items on the Personal Health Record (including the Health Power Assessment) within the plan year will receive a $150 incentive check from TML IEBP. <LI><B><A href="http://www.bewellattmliebp.org" target=_blank>BeWell at TML IEBP</A></B> - The BeWell at TML IEBP website (<A href="http://www.bewellattmliebp.org" target=_blank>www.bewellattmliebp.org</A>) contains exclusive programs and discounts available to TML IEBP Members. Some of the products included are Dental and Vision discount plans, special rates with popular physical fitness centers and debt management programs.
